Create one A4 image to illustrate the article 'Deadly data'. It uncovers failure to follow up on test results due to errors in electronic communication between hospital departments. In one such case the test results from CT scan in one department were never received by the department that had ordered the scan. This resulted in a 1.5 years delayed cancer diagnosis.
